A Magical Time of the Year

Dr. Hugh Tildesley

We are less than 1 month from Christmas…

Unbelievable!

Status Update: Blood work ok, my 12th course of poisons is in a few days. My oncologists are pleased with my progress, however as we go longer with this treatment, they become visibly concerned about me developing the neuropathy associated with Oxaliplatin. It usually shows up after 8 cycles, it looks like I may be playing with fire here, however, the incidence is around 10%, is reversible with time, and can be effectively treated with a number of agents. I will press on.

Tesla Titillations: Each week there are subtle changes to the car. Autopilot continues to improve. It operates best when the sensors are seeing 2 lane lines. It will operate following 1 lane line, unfortunately it is usually the lane separating you from oncoming traffic, and the car hugs that line and it can be quite unnerving and lead to knuckle whitening of any passengers who notice. My advice is only use autopilot with dual line input, the car stays perfectly centered and collective anxieties abate.
With 2 lane lines the car remains perfectly centered

One of my patients recently ordered a Model S 90,a top of the line model. I am going add this as one of the options on my prescription pad.

The Arts report: The Secret of Golf: The story of Tom Watson and Jack Nicklaus. Most golf fans know most of these stories but there were some nuances I had not appreciated. Firstly it appears Watson practiced more than Woods. Secondly he was an alcoholic and has fortunately stayed off the booze. Watson was instrumental in helping David Feherty get and stay sober.

As a coach for Donovan I always worried about the line between coaching and sabotaging a father son relationship.

Watson’s father was a prick, rarely giving positive feedback, commenting that every good shot was “lucky”. On the other hand Nicklaus’ father was as equally a good golfer as Watson senior and played frequently with Jack. The bets between them were always good-natured, such as a car or a house (this was when Jack was a teen!). They both knew the bets would never be paid but it taught competiveness. They usually walked off the course laughing.

Quite a contrast, but they both sought and achieved excellence.

Who can forget the Open in 2009, the 59-year-old Watson was 1 putt away from victory. Somehow the broadcast patched Nicklaus in by phone, his emotion and genuine cheering for Tom was palpable. The book tells us that Jack watched the complete broadcast, absolutely fixated. Generally, he never watches golf!

More touching was his support of Watson’s decision to putt from the fringe rather than chip on the 18 th hole that led to a bogey, his playoff with Stewart Cink which he lost. Watson finally accepted that it was the right play and his disappointment dissipated.

After reading this book I shot my best round in weeks , I focused on inner calm and focus.

15 Dogs: This year’s deserving winner of the Giller Prize. A great read and will leave you talking to your dog and expecting a conversation!

Celtic Lightning: compared to his previous book, “ How the Scots Invented Canada”, a huge disappointment. It became obvious that he had won a grant for the book and it is just not up to snuff. Read his previous book for a more comprehensive and organized look at our history and heritage.

Adele 25: a great album worth it.

The bucket list: get music throughout the house and be able to have different tunes play in different rooms.

Done: I purchased a sonos connect and 2 wireless speakers. I have hard wired speakers in Deborah’s study, the kitchen, the living room, and the patio. The wireless speakers can be moved but are now in the bedroom and my study. I can control the system with my iPhone. I have spotify integrated, thus music choices are endless, of course all my digitalized music is also on the iphone thus options are expanded.

A simplified diagram of the components
The E connect, this accepts input from your home network and some direct hardwire connections eg your CD player
The back view showing wired access
Great quality speakers that only need to be plugged in and get be moved where you want them.

Sorry to be so crass and materialistic …. But as Kurt Vonnegut wrote in Slaughterhouse-Five….

“ and so it goes”

Social: We approach the holiday season and the exchange of good wishes and good cheer. There is also the reality of crowded rooms and the pesky winter viruses. I must therefore pick and choose venues with caution and as many of you know I no longer shake hands but rather rub elbows. Also, my white count recovers about a week after the poisons ,thus I time my attendance at gatherings accordingly.

We recently had our staff Christmas lunch at C Prime, at the Century Plaza hotel, I have of course seen this hotel change over the last 30 years, I highly recommend the restaurant, comfortable ambience and very reasonably priced.

On December 8, I have my last official task as a member of the Point Grey Board as I open the AGM, this will end 8 years on the board, a highly satisfying activity. I will miss it.

We host our sometimes annual Open House on December 22, Hamish and Catherine return on the 21st, they are invaluable in helping Deborah bring it all together.

Christmas will bring Uncle Ted and Teresa as visitors; the Christmas dinner will be extra raucous.

Until next time…

A Month From the Winter Solstice

Dr. Hugh Tildesley
A short blog this time… sorry insomniacs.

The poison infusions went well, although each event is different, with subtle variability of side effects. Thankfully I am tolerating them well, for which I am thankful. Objective measures remain excellent, and on Wednesday I actually won some money from the band of bandits with whom I play golf. It has been a while since that happened!

Tesla Titillations: One of my patients this week, who has been following these pages, paid me a great compliment, stating that he finds the blog one of the few places where he can get comprehensive information regarding the car.

So what have I learned this week? We had some sub zero temperatures and I became concerned about slippery roads and not having the ability to downshift. In fact when you remove your foot from the accelerator, the electric motors quickly slow, there are of course no gears, thus the power and speed decrement are very controlled. The car has a true center of gravity as the 2 engines balance one another, thus forces are aligned to inhibit any loss of directional control. I have not yet experienced snow; stay tuned.

Power consumption goes up in the cold, this is to be expected, and however the car does effectively warm up instantaneously, which is a nice treat. After 30 years Deborah has graciously given over the garage thus frost on the windshield has not been a problem and not yet properly evaluated.

On Friday, November 20, we had our closing black tie dinner to celebrate the end of the golf season. Again Point Grey did a stellar job of food preparation, service and optimizing ambiance. The food theme had some Australian spin to celebrate Jason Day’s PGA win, which we wove into the evening. The attendance was the highest yet, and the evening raced by. Unfortunately 2 members did not appear in a tux and the usual fines were imposed with further sanctions to be discussed.

Arts Report: We saw a fabulous play at the Rothstein Theatre, the run was short and has ended, however, it has played in London and off Broadway, so if you come across it, it is a must see. It is all the more remarkable as it was Joshua Harmon’s first play.

We now move into the Christmas movie season, more reviews to follow.

Don’t worry be happy: It’s the little stuff that can give you a lift. We are now 1 month from the winter solstice…the days will be getting longer, seasonal blues should start to lift!

Trivia: What does Hockey Night and Canada have in common with the Hardy Boys and Nancy Drew”

Brian is on the right
Ans: The books were ghost written by Canadian Leslie MacFarlane, his son Brian was a host on Hockey Night in Canada, from 1964 to 1980.
Brian has written over 93 books, and is an accomplished painter.

Looking Forward

Dr. Hugh Tildesley
How am I doing: The blog is a little delayed, as I had to wait for my blood tests to come in. I have just completed 11 poison infusions.

The positive; weight remains steady; cell counts ok, tumour marker shows no significant worrisome trends.

I have been very successful in titrating insulin to control sugars while on dexamethasone, theoretically this will help to slow tumour growth.

I am now working 2, ½ days weekly, I generally nap those afternoons, waking quite refreshed.

I continue to bargain with Dr. Renouf to delay the next imaging, so long as I feel well and am maintaining my weight. To that end, a tentative CT will be booked for January.

My Practice: The patients have been turned over to Dr. Pawlowska, as has the office management. I continue to manage patients on-line.

The International Diabetes Federation meetings are in Vancouver this year, November 30-December 5.We have 2 posters that we will be presenting. In addition by the end of the month we will have the edits to the papers expanding on the posters submitted with final corrections. Intellectually therefore there remains ongoing stimulation.

What keeps me going? Looking forward to stuff.

At the end of this week will be our annual black tie dinner of the TWA group. Reception and dinner for 22, all seems nicely in order, full report next blog.

One of the highlights of my year historically has been our Christmas party. It is a lot of work for Deborah, thankfully the kids pitch in to make it all happen, and to minimize stress all around.

And of course Christmas. As yet unconfirmed we hope to have Uncle Ted and Theresa Gough here for both the Christmas party and the holidays.

The kids:

Donovan has had a very successful year and has surpassed his insurance sales goals. He is walking around trying to puff his chest out; this has been detracted by an increased abdominal girth. He now has his long time trainer, Yves, addressing this challenge! To be frank, he has unfortunately inherited my body habitus.

Great news for Hamish, he now has employment with EA (Electronic Arts), writing an interactive educational game. This will go a long way in securing a US visa, which he desperately needs. We could not be happier for him.

Catherine is finding second year law interesting; her improved marks reflect her hard work.

Tesla Titillations

A few weeks ago autopilot was downloaded and I have successfully adapted to it, so far without incident. One needs to follow the instructions, keep your hands on the wheel and understand its limitations.

Parts of the program were authored by Yogi Berra: “If there is a fork in the road, take it.” This of course is to be discouraged, hence the need to be vigilant.

Part of the download was to be auto park. It only started to work this week, and is in fact magical. Too early to see how often I will actually use it.

Classic Tesla Scream
There are now 2 ways to make women scream in the Tesla, in my sample, 2/3 will scream with the acceleration. The remainder will scream if you parallel park, no hands, and no legs and do not tell them what you are up to!

My agent has still not heard from Tesla, I thought by now I would be offered an audition for a commercial… bloody agent, I think I’ll start looking for a replacement.

More Kudos for Tesla Motors: For some reason my phone stopped synching. I missed getting into the car and reading what was on my calendar. My attempts to solve the problem were futile; I talked with customer support with a fairly quick resolution. From the time I initiated the call to talking to a human was 20 seconds! The support person was very polite as he asked to take over control of my car from Fremont, California. We reset the onboard computer 3 times by simultaneously pressing the 2 scroll buttons on the steering wheel; the ultimate solution involved introducing my iPhone as a “new device”. Great service, problem resolved, happy customer.

The Arts Report:

Spectre: Classic Bond cookie cutter format. Suspend all logic, understand he will escape every time and enjoy the chase scenes and explosions.

It’s what a movie should be… an escape and pure entertainment

Suffragette: a tail of sacrifice and bravery. No Academy Awards here but a story well told. At the end the dates that certain countries granted the vote an embarrassment.. Switzerland 1971! Give me a break.

Canada: variable dates by Province, in essence 1918.

Please take a moment to reflect on the innocent, who lost their lives in the senseless Paris attacks.
Until next time…..

Reflections on UK, home update

Dr. Hugh Tildeseley
How am I doing? Weight stable, averaging 165 pounds. My energy on the trip was exceptional, a little fatigued on returning home, running at about 90%.

On Tuesday (Nov 3), had another infusion of the poisons, it had been almost 1 month since the last treatment, my usual interval is 2 weeks. The beneficial effect has been a rise in my blood counts, this is protective against opportunistic infections, but I still “shake hands” with my elbow. I now taper my steroids more gradually and am working 2 , ½ days per week, rather than every 2nd week. I am so far managing quite well.

Frequently unasked questions: What is the biggest drag in this whole journey?

Simple, the unknown, but upon reflection CA or not who can tell the future.

Has there been a benefit?

Yes. To have my family rally around me is beyond description. There are perhaps medical reasons for my progress (chemo, blood sugar control, genetics of having a thin vs. thick connective tissue surrounding my tumours), but there is also the power of a lot of people having my back, their well wishes, and prayers are not to be ignored.

I have just finished my 10th treatment; I plan to continue until Christmas with a re-evaluation at that time. I really do not plan anything outside of 6 weeks in advance.

The last day in the UK:

Sleuthing in Watson's Study
We had to be out of the apartment by 10 AM, our flight to YVR did not leave until 5:25 pm, thus a few hours to kill. We made our way to the Holmes museum, no school kids this time and an interesting walk through. Rather neat that it was possible to sit in some of the rooms, a thorough search for Sherlock’s drugs was unsuccessful.

Some biking observations: No super bike lanes in this city, please compare practical use of space vs. the Vancouver nonsense.

London bike lane marker
Prudent cycling in London
Vancouver bike lane, eliminates a full lane of traffic creating havoc and traffic jams at lights...scandalous

The curved doors can trap the head of an unsuspecting passenger

Watch out for the tube’s closing doors; note the curve at the top. My noggin got caught in a crowded train; extraction successful but a worry as I am on blood thinners. No harm, no foul.

We had stored our bags at Victoria station, the walk back from 22 Baker Street perhaps a little long but good preparation for the upcoming flight. We took the tube to Heathrow, a bit of a bother getting down stairs but from then on very easy, especially transferring to the Piccadilly line at Hammersmith. The golf bag storage a bit userous, however I could not imagine toting the bag to and fro London. We landed on time and were 1 hour from landing to getting in the front door.

Tesla Titillations: Note Donovan’s birthday present has arrived. For those who do not read every word of these ramblings let me take you back in time: The September 5 post

“On Wednesday’s men’s night we were on the second tee and the back shop guy was confused where to put the tee marker, I suggested a quick call to the pro shop for clarification. The conversation his end started: “ I am with Dr. Tesla’s group and was…” Classic. I do not mind his mispronunciation of my name. “ Donovan liked the description thus a new plate for the car :

The auto pilot download is working fine, prudence is the key, self-parking is problematic, and a new download tonight may fix it. Not an issue for me, other than getting the product that I paid for!

Liam Kent in Action
You may recall on a recent Tesla trip we visited Deborah’s nephew, Liam Kent, at George Fox University. He is on a baseball scholarship there, a solid left-handed pitcher. At our lunch together he confided to us his goals and wishes for the coming year. It seems he is ahead of schedule, having made the team and likely as a starter. In addition the West Coast League, Bellingham Bells, have made a 10-day try out offer. This is the league where many of the college players play in the summer. Nice feather in his cap!

The rest of the month is filling up, Deborah’s sister , Susan is visiting for a few days, Catherine returns on Tuesday for a short visit and belated birthday celebration. Hamish recorded a beautiful song for her, check it out. ( copy and paste into your URL, then play)

The London High Commission

Dr. Hugh Tildesley
George arranged a private visit for us at Canada House. We were met by Brigitte Duchesne, who over the next 90 minutes gave us a very informative tour and a summary of much of
the artwork.

The Trafalgar Entrance

A brief History: The building was constructed between 1824-1827, originally 2 buildings housing the Union Club and the Royal College of Physicians.

The Canadian government purchased them in 1923 and Canada House was officially opened by King George in 1925. The King George Chair commemorates this opening although he never sat in it.

The King George Chair

Recently extensive renovations were completed and all consular work was consolidated to this address. This allowed for the selling of Macdonald House in Mayfair. When the balance sheet is examined, extensive renovations, and the purchase of an adjacent building ended in a net profit! Operations are in part by a tenant paying $3,000,000 annually in rent.

The renovations included extensive electrical work, and remodeling. There are meeting rooms for each province and territory, the square footage of each room proportional to the provincial population. All of these rooms have a provincial flair and coloring as well as representative artwork.

The top floor has a large reception/dining room with a kitchen adjoining. Only Canadian beer, wine and spirits are served at official functions.

The edifice makes one proud to be Canadian, as we put our best foot forward to meet the world in one of the world's most important cities.

We are now safely home, a further edition of the blog on Friday or Saturday, as I recover from Chemo early this week

Until  next time.......
